Returning Students, any program site - Use this WWU Applicaton
All returning students should include a personal statement explaining the reasons for leaving and returning to Western as well as an academic plan of study which outlines the courses to be taken.

Current WWU, Transfer, and returning students must complete each segment of the
Human Service application below.
Incomplete applications are not considered.

1. Human Services Program Application
2. Professional Expectations Agreement
3. Essential Functions Self Assessment
4. Technology Self Assessment Statement
(Distance-Learning applicants only)
5. Criminal Conviction Disclosure form.
Most prior convictions do not prevent someone from entering the program. See, Prohibited Convictions.
6.  Once an applcant has been offered admission to the program he/she must initiate and successfully complete a background check.  Successful completion of the background check.   Applicants are responsible for this expense (approximately $55.00) and will be able to view their results online. In some rare instances, fingerprinting may be required ($0-$20).   Failure to successfully complete the background check process will result in the disruption, delay, and/or denial of the students' ability to continue in the major.  In the event questions arise about your background check results, you will be contacted by the program manager or department chair.

Applicants concerned about their personal history and whether or not it will disqualify them from being accepted to the program should review the Prohibited Convictions page and are encouraged to initiate a background check prior to applying to the program.
